Histamine Release Assay

A histamine release assay measures histamine secretion from immune cells to evaluate allergic reactions, hypersensitivity or biological activity.

Histamine release assays quantitate mast cell and basophil degranulation in response to pharmaceutical exposures detecting potential anaphylactoid reactions. Whole-blood basophil activation tests measure cell surface activation markers following drug exposure. Flow cytometry identifies activated basophils expressing upregulated CD63 and CD203c. Assay results predict anaphylaxis risk aiding dose escalation decisions in early clinical trials.

Pharmaceutical applications include monoclonal antibody and complement activation inhibitor assessment where direct mast cell activation risks exist. Positive histamine release predicts need for premedication or modified dosing regimens. Desensitisation protocols developed for positive-testing patients gradually escalate doses permitting therapeutic use. Baseline assay variability requires careful interpretation avoiding false-positive conclusions. Regulatory acceptance of basophil activation tests supports replacement of animal-based histamine release methods.
